The CG-719B is the main application for a U.S. Merchant Mariner Credential. It looks like ordinary paperwork, but a small mistake can create extra questions or an incomplete package.

The goal is not to finish the form quickly. The goal is to submit a clear, consistent application that tells the National Maritime Center exactly who you are, what transaction you want, and which endorsements you are requesting.

This guide helps you perform that review. Always download the current form and follow the current NMC application instructions before submitting.

Mistake 1: Using an Old Saved Copy

Forms, submission portals, and instructions can change. A CG-719B stored on your computer from an earlier application may no longer be the best copy to use.

Begin on the official NMC website each time. Download the form, save it with a clear file name, and then complete it.

The NMC currently tells applicants not to complete the CG-719B inside a web browser. Save it locally and use Adobe PDF software so form data and signatures are less likely to disappear.

Mistake 2: Letting Names Disagree Across Documents

Your legal name should be consistent across the CG-719B and supporting records. Watch for:

  • Missing middle names or initials
  • Previous names or aliases not addressed
  • Hyphens and suffixes used inconsistently
  • Nicknames on course certificates
  • Transposed first and last names
  • Different names on ownership records

If a record legitimately uses another name, include the explanation or documentation required by the current instructions.

Mistake 3: Requesting an Unclear Endorsement

"Captain's license" is not a precise endorsement request.

The application needs the credential and limitations you seek, such as OUPV or a particular Master route and tonnage. If you want more than one endorsement, make the request unambiguous.

Use the official NMC checklist page to identify the correct title. Compare it with your sea service and training before entering it on the form.

Mistake 4: Selecting the Wrong Transaction

Original, renewal, raise of grade, modification, duplicate, and replacement are not interchangeable.

Choose the transaction that describes what you are doing. For example, moving from OUPV to a qualifying Master endorsement may be handled differently from replacing a damaged credential.

The wrong choice can affect the documents and fees expected in the package.

Mistake 5: Leaving Required Fields Blank

Review every section that applies to you. If the instructions permit "N/A" for a field that does not apply, use it rather than leaving the evaluator to wonder whether you missed the question.

Pay special attention to:

  • Contact information
  • Citizenship and identity fields
  • Mariner reference number, when applicable
  • Requested endorsements
  • Conviction and safety-and-suitability questions
  • Oath, consent, certification, and signature sections
  • Dates next to signatures

Do not add information the form does not request, but do not skip applicable blocks.

Mistake 6: Treating Sea Service as One Number

The CG-719B is not the place to solve a disorganized sea-service history.

Prepare your supporting sea-service evidence first. For small-vessel service, the CG-719S may be appropriate. Official NMC guidance for completing the CG-719S says an individual form must be completed for each vessel on which service is claimed.

Make sure the endorsement requested on the CG-719B is supported by the vessels, routes, dates, roles, and service in the attachments.

Mistake 7: Mixing Medical and MMC Submissions

The medical certificate and Merchant Mariner Credential processes are connected but handled separately.

The NMC application page warns that medical certificate applications submitted through the MMC portal will not be processed. Follow the current medical certificate instructions for the medical application and the MMC instructions for the credential package.

Save confirmation for both.

Mistake 8: Forgetting a Required Supporting Item

Depending on the transaction and endorsement, your package may need:

  • Sea-service evidence
  • Course certificates or assessments
  • Drug-testing evidence
  • TWIC or TWIC enrollment documentation
  • Citizenship or identity records
  • Proof of vessel ownership
  • User-fee receipt
  • Conviction statement or supporting records, when applicable

Do not use a generic internet list as the final authority. Check every line of the official checklist for your endorsement.

Mistake 9: Sending Poor Scans

An evaluator cannot approve what cannot be read.

Before upload, open the final file and check:

  • Every page is present
  • Text and signatures are legible
  • Pages are right-side up
  • No edge is cut off
  • Checkboxes and typed fields display correctly
  • The file opens without a password
  • The file stays within the portal's current size limit

The NMC currently instructs most MMC applicants to combine relevant PDFs into a single submission rather than uploading the CG-719B one page at a time.

Mistake 10: Submitting Without a Final Cross-Check

Use two reviews:

Form review: Read the CG-719B from top to bottom.

Package review: Compare every attachment against the official checklist and the NMC's application order for review.

Confirm that the same legal name, vessel facts, dates, and endorsement request appear throughout the package.

Keep the Submitted Version

Save the exact PDF package you send, the fee receipt, the submission confirmation, and every message received afterward.

If the NMC requests additional information, you will be able to see what the evaluator already had and respond accurately.
